Open questions. The receptor pharmacology is well characterised and the honest headline is that nobody yet knows which arm of it does the work. Selective calcitonin receptor activation is reported as ineffective, non-selective agonism appears better, and a competing compound with a different balance outperformed cagrilintide in the one published head-to-head — in a paper written by that competitor's developers.
Amylin and calcitonin receptors are also expressed well beyond the tissues of metabolic interest, calcitonin signalling being centrally involved in bone and calcium handling, so a non-selective agonist has effects wherever the family is expressed.
