MOTS-c is a 16-building-block peptide whose instructions sit inside a mitochondrial gene, which is what makes it structurally unusual: the recipe lives in mitochondrial DNA rather than in the cell nucleus. The mechanism described in the founding work is indirect and specific. MOTS-c blocks the folate cycle and the purine building attached to it, a molecule called AICAR piles up as a result, and AICAR is itself a natural switch for AMPK, one of the cell’s main energy sensors.
So the route runs folate cycle to AICAR to AMPK, not direct binding to the sensor. Later work reports the peptide moving into the nucleus when the cell is under metabolic stress and acting on gene activity there, which is unusual for something encoded in mitochondria.

The founding result is a prevention experiment: mice got the peptide alongside a high-fat diet from the start, which is a different question from reversing something already established, and the two get run together constantly when this compound is described.
