PT-141, bremelanotide, is a ring-shaped seven-building-block relative of alpha-melanocyte-stimulating hormone that switches on the melanocortin receptors MC3R and MC4R. The mechanically interesting point is where it acts. MC4R sits densely in the brain and spinal cord, and the pathway bremelanotide engages is a nerve pathway rather than a blood-vessel one, which makes it a categorically different thing from the PDE5 drugs it gets compared with: those work on muscle and blood flow out in the body, this works on signalling in the brain.
It is a close relative of melanotan II, differing at one end, and that difference greatly reduces its activity at MC1R, the receptor behind pigment production. That is why bremelanotide does not carry melanotan II’s tanning profile. It was approved in 2019 as Vyleesi for low sexual desire in premenopausal women, making it one of only two approved medicines in this catalog.

Open questions. The receptor identity is settled and the brain-rather-than-blood-vessel mechanism is well supported. What gets said less often is that the evidence for the effect is largely what people reported about themselves. In the study above the self-reported measure moved and the instrument-measured one did not, and that pattern repeats across this literature. It does not make the finding false, but it is a materially weaker kind of evidence than a physical measurement, and readers should know which one they are being shown.
The approved use is narrow and specific, and the trials behind approval were run in that group rather than in people generally. Melanocortin receptors are not confined to one tissue, which is why nausea and flushing are the characteristic effects of this class rather than accidents. As with the other approved molecules here, the licensed product is a manufactured medicine with release testing behind it, and a research vial carrying the name is not that object.
