SS-31 is a man-made four-building-block peptide from the Szeto-Schiller series, and it turns up in the literature under several names including elamipretide, MTP-131 and Bendavia. Its mechanism is unusual because the target is a fat molecule rather than a protein. It sticks to cardiolipin, the negatively charged fat in the inner membrane of mitochondria that the folded shelves inside them depend on. Its positively charged building blocks are drawn to cardiolipin’s charged heads and its bulky ones tuck into the greasy tails.
Since cardiolipin gathering in one spot helps the membrane curve the way those folds need, sticking to it is proposed to hold that structure together. The alternating bulky-and-charged pattern is what carries the peptide through membranes and concentrates it in mitochondria; it is a design feature of the series rather than an accident.

The biophysics is well described and the animal work is consistent, but elamipretide has been through human trials under a sponsor and the results across different conditions have been mixed rather than uniformly good. Any description of this compound that reports only the favorable animal picture is picking and choosing.
