Open questions. The biochemistry of NAD is textbook material and the age-related decline is well documented. The open question is whether supplying NAD from outside a cell changes anything inside it. NAD does not cross the membrane freely; outside the cell, enzymes break it into precursors, which are then taken up and rebuilt. That makes giving NAD an indirect and inefficient way of delivering precursors at best, and it is the reason the research field works with nicotinamide riboside and nicotinamide mononucleotide instead.
Beyond that: most of the lifespan findings are in mice and other short-lived animals; human trials of NAD precursors have generally shown that the markers in blood can be raised while the clinical results have been far less consistent; and CD38 activity climbs with age, meaning the pool is being drained faster at exactly the point supplementing is proposed. Anyone comparing products in this category should ask what is actually in the vial and by what route it is supposed to reach a cell.
