Ipamorelin is a five-building-block peptide that came out of a chemistry program which stripped a two-building-block piece out of the middle of an older compound, GHRP-1. It works on the ghrelin receptor, the same one the body’s own hunger hormone uses, rather than on the GHRH receptor, and what makes it interesting is selectivity rather than strength. Earlier compounds in the GHRP series release growth hormone but also push up ACTH and cortisol, which is the practical problem with that class.
In pigs, ipamorelin released growth hormone about as well as GHRP-6 while leaving ACTH and cortisol no higher than a plain GHRH challenge produced, and that separation held at doses more than 200 times above the amount needed for half its growth hormone effect. It also left FSH, LH, prolactin and TSH alone. That is the specific claim that made the molecule notable: a compound on the ghrelin receptor with the selectivity of GHRH.

Open questions. The selectivity claim is well supported and it is the most interesting thing about this molecule, but three limits are worth stating. The release seen in the human work is a single burst that returns to baseline, so any account implying a sustained rise is not describing what was measured. The rat bone work found the bone growing while IGF-I and the bone turnover markers stayed put, which is an unresolved inconsistency rather than a supporting detail, and it usually gets left out.
And unlike the incretin peptides here, ipamorelin never finished development into an approved product, so there is no large modern clinical record and no official reference standard behind the name. Most of the primary work dates from the late 1990s.
