Both changes exist to reduce binding to the IGF binding proteins, which in circulation sequester the great majority of IGF-1 and limit how much reaches the receptor. Open questions. The structure is well defined and the reduced IGFBP affinity is real. The inference usually drawn from it — that this analogue is therefore more potent than IGF-1 — is not supported by the comparison above, where it was the least active of three in one assay and inactive in another. That is one paper in bovine cells and should not be over-read either, but it is the head-to-head data that exists and it points the opposite way to the marketing.
