Semax is a seven-building-block peptide made from a fragment of adrenocorticotropic hormone with a three-building-block tail added on the end. That tail is the design feature: it sharply slows enzymes that would otherwise clear the sequence almost immediately. The parent fragment was picked because it carries the behavioral and nerve-supporting activity of ACTH without the part that drives cortisol, so Semax does not push cortisol up the way ACTH does.
The proposed mechanism is not sitting on a receptor in the usual sense but changing how much of certain nerve growth factors get made, specifically BDNF and its partner receptor TrkB in the hippocampus. It is registered and prescribed in Russia and is not approved in the United States or the European Union.

Open questions. The molecular work is real and repeatable enough to describe, and it is also narrow: single doses, in rats, measured hours later, with changes in the range of 1.4 to 3 times rather than the transformations sometimes implied. The step from a change in hippocampal BDNF to anything a person would notice is not established, and the rodent measure used here is learned avoidance, which is not a test of thinking in the everyday sense.
Nerve growth factor changes are region-specific and include at least one decrease, so saying Semax raises nerve growth factors oversimplifies the authors’ own conclusion. The wider limit is judging the work at all: much of this literature comes from a single institute and a good deal of it is published in Russian, which makes independent replication harder to assess than the citation count suggests. Approval in Russia is not evidence of effectiveness by United States or European standards, because those standards were not applied.
