Status: No FDA-approved application. This compound is sold as laboratory material. Where research on it has been published, it sits at the animal, cell or early-human stage, and this record does not summarize what it found. A supplier is not a neutral judge of evidence about its own catalog.
Long R3 IGF-1 is human IGF-1 with arginine substituted for glutamate at position 3 and an N-terminal extension of thirteen residues, Met-Phe-Pro-Ala-Met-Pro-Leu-Ser-Ser-Leu-Phe-Val-Asn. Both changes exist to reduce binding to the IGF binding proteins, which in circulation sequester the great majority of IGF-1 and limit how much reaches the receptor. The reasoning is straightforward and the binding data support it.
What does not automatically follow is greater potency: in a published head-to-head comparison the analogue was less active than native IGF-1 in one assay and failed to reach significance in another where native IGF-1 and des(1-3)IGF-I both succeeded. Escaping the binding proteins changes distribution and clearance as well as availability, and those can work against each other.

Open questions. The structure is well defined and the reduced IGFBP affinity is real. The inference usually drawn from it — that this analogue is therefore more potent than IGF-1 — is not supported by the comparison above, where it was the least active of three in one assay and inactive in another. That is one paper in bovine cells and should not be over-read either, but it is the head-to-head data that exists and it points the opposite way to the marketing.
Beyond that: no human data, no approved product, and the compound's established role is as a cell-culture reagent, which is a legitimate research use and a different thing from a therapeutic candidate. IGF-1 receptor agonism is also mitogenic by nature, and the long-term consequences of unopposed signalling are the reason this axis is approached cautiously in drug development.
