It works as a decoy, mopping up myostatin and related ligands before they can signal. That mechanism is deliberately broad — activin type II receptors bind several TGF-beta superfamily members, not myostatin alone — and the breadth is the likely explanation for what happened in the clinic. The compound produced effects outside muscle, and those effects, rather than any lack of muscle activity, ended the programme.

Open questions. The defining fact about ACE-031 is that its clinical trial was halted early over safety, specifically nosebleeds and telangiectasias — small dilated vessels visible in the skin. Those are vascular findings, not muscle findings, and they are consistent with the decoy binding TGF-beta superfamily ligands beyond myostatin. Any account of this compound that reports the lean-mass and bone-density trends without the discontinuation is describing half of a published result. The efficacy signals themselves were trends and the walk-test difference did not reach significance.

There is no approved product, and the programme did not continue. Note also that this is a large Fc fusion protein rather than a peptide, which has consequences for handling, stability and what an identity assay on a lot would even involve.
